Miscellanous Notes

There are no charges for medications or surgical supplies. These are included in the surgical fees. A $600.00 deposit is required to secure your appointment date. The balance of the fee for each session is due at the time of service. Personal checks are not accepted unless received in advance and have cleared prior to the surgery.
